Parish Registration Form

Welcome to St. Thomas More! To register at the parish, please fill out the form below. This is preliminary information. A follow-up message from the rectory will help finalize your parish registration. Please have the following information ready:

• Your sacramental information (dates, parish, religion if not Roman Catholic)
• If married, spouse's name, birthdate, contact info and sacramental information
• If you have children, their names, birthdays, sacramental information, current school information.
• Sacraments you or your family need to still receive.
• Would you like to participate in our online giving program or recieve traditional envelopes?
• Possible ministries of interest (liturgical minister, parish groups, etc...)
• Any questions you may have about the parish.
